Per Diem Hospice Nurse Practitioner/CNP/NP Job

HCR ManorCare provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy.

The Nurse Practitioner will provide on-going medical management and acute interventions for patients in the hospice and palliative setting. The NP will work as a member of the interdisciplinary team to effectively designate processes for meeting the medical, psychosocial and spiritual care needs of the patient. The NP will develop updates and evaluate medical treatment plans, medications, and other necessary care.

- At least one year in Hospice/Palliative care
- Prior Nurse Practitioner experience, preferred
